Trusted Assessor
We are looking for a Trusted Assessor to support adults with care and support needs, and their families, to identify what matters to them, understand their options and access the right support to live as independently as possible.
This is a role where you will combine assessment, problem solving, partnership working and relationship building to help people achieve the outcomes that matter most to them.
The role will include:
- Carrying out strengths-based Care Act Assessments and reviews
- Developing person-centred support plans with individuals and families
- Providing clear information about care, support and local services
- Working alongside health, social care and voluntary sector partners
- Maintaining accurate records and contributing to quality and performance monitoring
Closing Date: 9am Monday 7th September
*An enhanced DBS will be required for each successful candidate, paid for by Bexley Mencap (T&Cs apply)

Skills, Abilities and Knowledge
- Excellent communication and interpersonal skills
- Ability to build rapport with people from diverse backgrounds
- Ability to explain complex information clearly and accessibly
- Ability to prioritise and manage a varied caseload
- Ability to work collaboratively with professionals while maintaining the person’s voice at the centre of decision making.
- Good written communication skills, including the ability to produce clear assessment reports and support plans
- Strong IT skills including Microsoft 365 and case management systems
- Knowledge of safeguarding, confidentiality and data protection
Experience
- Experience of working in adult social care, health or support services
- Experience of supporting adults with learning disabilities, autistic people and/or other adults with care and support needs
- Experience of record keeping, monitoring or audit processes
